Maryland’s new Total Cost of Care Model holds the promise to deliver better health to people along with an economically sustainable health system. Through global budgeted revenue for hospitals, hospital financial accountability for attributed Medicare beneficiaries, aggressive quality and population health goals, and tools to harmonize care delivery practices with non-hospital providers, the model aims to control cost growth statewide. For home care providers, this will mean opportunities to partner with hospitals in executing efficient care delivery across the continuum.
