Home health agencies will gain a better understanding of the palliative care model of care, how palliative care can decrease overall healthcare expenditures across the continuum of care and improve population health management.  


Objectives:

  1. Define the scope and role of palliative care for individuals with serious illnesses.

  2. Discuss the palliative care model of care.

  3. Illustrate how the palliative care model of care decreases overall healthcare expenditures across the continuum of care and improves population health management.

 

About the Speaker


Timothy G. Ihrig, M.D., M.A.

Timothy G. Ihrig, M.D., M.A. is Chief Medical Officer of Crossroads Hospice and Palliative Care. He also is founder and Chief Executive Officer of Ihrig MD & Associates. Dr. Ihrig is a palliative care physician. He cares for the most vulnerable and sick individuals. He creates and leads health systems and teams through change that care for the most seriously ill people. You will be contacted so that we may collect the names and email addresses of all staff who will be participating in the webinar.


Participating companies will receive an email confirmation after they register with the webinar link and instructions on how to log in to the webinar.


In order to receive a Certificate of Completion, each participant must:
1.    Attend the entire webinar.
2.    Complete an online evaluation form.
3.    Complete, sign and submit a half sheet for each participant with the Last four digits of Social Security Number; First & Last Name; Birth Date; Address, City, State, Zip; Phone Number; Email Address; License/Certification Number; and Signature.

Upon receipt of this information, a certificate of completion will be emailed to each participant.
